The information I have provided in this Application for Employment is true, correct and complete. False, incomplete or misrepresented information of any kind will be su cient cause for my application to be rejected or, if discovered after I am employed, cause for immediate termination of my employment.

I authorize Gootee Construction, Inc. to contact and obtain information about me from my previous employers, educational institutions and “references” I provided and any other party necessary to verify the accuracy of information I disclosed in this application, a related employment resume or a personal interview. To assist in the processing of my application, I waive all rights and claims I may otherwise have against Gootee Construction, Inc. or its representatives for seeking and using information to evaluate my employment request and all other persons, corporations or organizations who provide information for this purpose.

This application will expire in 30 days. After that date, unless otherwise notified, I understand that my status as an applicant will end. I may re-apply in the future by completing a new application.

I agree that, if I am employed, I will abide by all the rules and regulations of Gootee Construction, Inc. I understand that the taking of a drug and alcohol tests, when given pursuant to company policy, are a condition of continued employment and refusal to take such tests when asked will be grounds for immediate termination. I also understand that my employment is “at-will” and may be terminated by myself or Gootee Construction, Inc. at any time for any reason at all, with or without prior notice. I further understand that no employee of Gootee Construction, Inc. is authorized to enter into any written or verbal employment contracts with me for any definite period of time without the express written consent of the President of Gootee Construction, Inc.
